Sports Participation and Physicals
If your child has a PCM in the Medical Group and needs a sports participation form:
Call the Appointment Line to send a message to your PCM team or send a Secure Message via the MHS Genesis Patient Portal requesting a sports participation form, and in 2-3 business days you will be contacted by the Pediatric Clinic.
•If your child has had a Well Child Visit within the last 12 months, a sports participation form can be provided without the need of an additional appointment.

•If your child has not had a Well Child Visit within the last 12 months, your child will need an appointment scheduled before a sports participation form can be provided.
Due to the high volume of patients needing appointments please contact the Pediatric Clinic at least 6-8 weeks prior to the start of the sports program to ensure your child can participate without delay.
If your child does NOT have PCM in the Medical Group but has a GP off base:
In accordance with the Hospital’s SPACE-A policy, parents should contact the Appointment Line 01638528010 at 1200 or after to try to get their child a same day appointment. If no appointments are available, parents will need to call back each day at 1200 or after. As long as your child does not need to be further evaluated by the GP for any medial concerns, a sports participation form will be provided at the end of the appointment.
Alternatively, the Cambridge Private Doctors Group is able to complete sports physicals; however, the cost of the appointment will be 164 pounds. Parents can submit a claim with their insurance for reimbursement. Students MUST get a vision screening prior to this appointment and bring sports participation form with vision screening results with them. Cambridge Private Doctors Group can be contacted at 01223 967995. Talk to the school nurse about getting a sports participation form and a vision screening completed.
Remember:
According to the American Academy of Pediatrics, seeing a pediatrician for a sports physical only is not enough.
However, we understand your unique situation where GP/NHS care differs from ours, so we do want to ensure a safe process to allow your child to participate in base sporting activities.
